Making a Freedom of Information Act Request

 
     

There are two methods to request NETCOM related information using FOIA:

  1)

Complete the Request Form using the link below. The form can be submitted online or printed.

Submit a letter including your full name and address, phone number, e-mail address, a reasonable description of the record(s) requested, and a statement of your fee category and your willingness to pay applicable fees. A link to a sample letter is provided below.
